Assessment 2 WHS Workplace Incident Report

Instructions:
In a group of up to four, identify and research a fatal workplace incident. The incident must have occurred in recent years (2017 – 2020). Use reliable sources of information, academic sources (e.g. books and journals) and secondary sources (e.g. media articles, official reports) are acceptable. Work together in your group to create a detailed account of this critical incident and present the case in a clear and objective manner.
Following your explanation of the fatal workplace incident, outline the active and latent factors by considering James Reason’s Swiss Cheese Model (refer to theory, assigned lecture readings, other readings from the academic literature and any relevant legislation to justify your application of the Swiss Cheese Model). Then outline best practice hazard control measures that you would use and any system-based improvements required to prevent this incident from occurring again.
Finally, as though you were government officials investigating the critical incident (e.g. Safe Work NSW), propose some recommendations on immediate actions required to reduce the potential for a repeat incident and by outlining a suitable punishment or consequence for the employer (or any other relevant stakeholders). Consider the moral and ethical behavior of the stakeholders in your recommendations. Justify all your recommendations by using relevant literature, and referring to similar or relevant scenarios.
A ‘Similarity Report’ will be produced by Turnitin when you submit your group report. Resubmit the revised versions until you are happy with the similarity report. If you resubmit your work after making changes, the similarity report takes longer (24 hours) to process. Be sure to allow extra time in your group’s work plan to ensure you submit on time. Penalties apply for late submissions.
