2. Complete the document titled Process Evaluation Planning Worksheet
3. Use your logic model to inform your responses
4. Consider timing, how you will collect the data, and how you will use the data to inform the future of your program
5. Add more rows to each document if needed to thoroughly build an effective evaluation plan
6. Demonstrate you have thoroughly thought through your evaluation plan to ensure you understand the success of your program and the causes of its success in order to know what to change or keep consistent the next time it is implemented
